Reablement

What happens after hospital discharge?

How reablement helps people rebuild confidence at home and reduces the risk of readmission.

4 min read

Leaving hospital is a relief — but the first few weeks at home can be uncertain. Reablement is short-term support designed to help people regain confidence and independence after an illness, injury or hospital stay.

What reablement involves

  • Help with daily routines in the early days at home
  • Gentle encouragement to rebuild confidence with everyday tasks
  • Support with medication and meals
  • A plan that reduces as independence returns

Why it matters

Good reablement helps people do more for themselves again, rather than becoming dependent on ongoing care. It also reduces the chance of being readmitted to hospital.

How long does it last?

It is usually short-term — often a few weeks — and is reviewed regularly. If ongoing support turns out to be needed, that can be arranged as the plan adapts.
